Kristen Mucitelli-Heath

Senior Vice President, Population Health and Advocacy, Trinity Health NY

Kristen Mucitelli-Heath is the Senior Vice President, Population Health and Advocacy for Trinity Health NY (comprised of St. Joseph’s Health, Syracuse and St. Peter’s Health Partners, Albany). In her population health role, she oversees operations of two regional Clinically Integrated Networks (IHANY and CNYAIM) as well as Concordia Health, a Super (multi-region) CIN and the population health strategy and operations for two health systems and over $1B in contracts. Kristen also oversees Community Health Connections, one of the state’s largest Medicaid Health Home programs including more than 4000 members over 12 counties.


In her advocacy role, Kristen leads engagement and advocacy with all levels of government, building off more than 20 years’ experience in policy and government relations.


Prior to her 14 years at Trinity, Kristen was Executive Director of the Upstate Caucus in the NYS Senate; Chief of Staff/VP of Planning to two Commissioners at Empire State Development Corporation and served two NYS Governors in various roles for the Executive Chamber.


She currently serves as a board member on the national Healthcare Transformation Task Force (HCTTF), Rome Health, CNY Works, The Plaza Corporation, and the Concordia Healthcare Network SuperCIN.
